进行事故调查、找出事故发生的真正根源，从而杜绝类似事故的再度发生是现代HSE管理的重要环节。详细、透彻的事故调查是找出事发根源的前提条件，事故分析表是根源分析的一有效工具，本课程将介绍事故调查的基本原理，如何收集现场证据、跟证人的面谈技巧，以及如何利用事故根源分析表来进行事故根源分析，并制定整改措施。 本课程是为那些参与事故调查的人员而设计的，包括编写事故调查报告的主管领导、生产及维修监督、安全监督及安全工程师等。

Accidence Investigation and Analysis
Course duration: One Day
Conduct incident investigation, to dig out the root cause of an incident, so to prevent similar
incidents from happening again is one of the important elements for modern HSE management. A
thorough investigatio n is the basic requirement of root cause analysis. Comprehensive List of Causes
(CLC) or Fault Tree is a powerful tool for conducting root cause analysis.
The course will introduce the basic principles and process of incident investigation, including how
to collect the first-hand material, witness interview skills , how to use CLC and how make
correction actions.
The course is specifically designed for those who will be involved with an incident investigation
such as managers , supervisors and HSE specialists
